1.       PURPOSE OF JOB

1.1               To play a lead role in the development of good community care practices and procedures

1.2               To work in conjunction with the office care co-ordinator and to assist in the smooth running and upkeep of all care delivery practice and procedures.  To cover the office and rosta in the absence of the care co-ordinator during Annual Leave and sickness absences

1.3               To provide a cover care service and deliver care as instructed by the care manager and care co-ordinator when emergencies, short term requirement and covers as needed.

1.4               Full-time post; office working hours 9am to 5pm – flexibility hours required outside of these times plus out of hours service as and when required along with on-call duties

2.       PRINCIPLE RESPONSIBILITIES

QUALITY AND STANDARDS

2.1               Deliver initial care service to all new service users, to introduce permanent carers and to monitor and review ongoing packages of care, undertake regular spot checks, assessments and review visits as required.

2.2               Attend case conferences or reviews as required in order to monitor and review care packages.

2.3               To further establish the quality reputation of our client and identify opportunities for increasing service provision , through effective liaison with other agencies as appropriate:

-          Care Managers

-          Home Care Organiser

-          Purchasing Officers

-          Hospitals

-          GPs

-          Friends

-          Relatives

-          Other interested parties

2.4               To participate in staff meetings on a regular basis

2.5               Promote the development of staff

2.6               Attend and participate in training as required for service development. Contribute to service of development of appropriate training programmes for staff.

2.7               Develop the service, putting Equal Opportunity Policy into practice, ensuring that the service provided is sensitive to need and is culturally appropriate.

2.8               To introduce the service to all new users, ensuring they are given and have explained all relevant information, including the customer charter and complaints procedure.

2.9               To carry out risk assessments at all new service users houses and ensure the working environment is safe for carers and Service Users, where to liaise with social services to rectify unsafe areas, items and to inform carers, Service Users accordingly.

2.10           To deputise for the co-ordinator in his/her absence or when co-ordinator is deputising for care       manager

3.       ADMINISTRATION

3.1   Ensure that client/staff files are maintained and updates as required. Ensure the events and outcomes of client visits, care consultations, spot checks, complaints etc., are all recorded in all cases immediately following the event/visit.

3.2   Ensure that Service Users care plans are updated, completed regularly and accurately by carers and kept in the client’s home. Copies of clients notes over 6 weeks old should be returned to the clients file in the office.

3.3   Preparation of statistics / reports on service delivery as directed by the care manager / office co-ordinator.

3.4   To take part in consultation and planning meetings with care manager / office co-ordinator as required.

3.5   To answer telephone enquires, ensure correct message recording, actioning of messages and the maintenance and upkeep of daily records information including key log, ID, accident book etc.

3.6   To participate on the emergency out of hours service as required by the Branch Manager / Deputy Branch Manager or care co-ordinator.
